NDIS Reports
Our team brings a wealth of experience in working within the NDIS sector, always advocating for the best outcomes for participants.
NDIS Functional Capacity Assessment
An NDIS Functional Capacity Assessment (FCA) conducted by an Occupational Therapist is a comprehensive evaluation of a participant’s ability to perform daily activities and participate in various aspects of life, such as self-care, mobility, communication, and social interaction. The Occupational Therapist uses clinical observations, interviews, and standardized assessment tools to assess the participant’s physical, cognitive, and psychosocial functioning. This assessment provides an objective understanding of how a person’s disability affects their functional abilities and what supports are needed to help them live more independently and achieve their goals.
For NDIS participants, a Functional Capacity Assessment can play a vital role in supporting access to appropriate funding and services. The detailed report generated from the assessment helps inform NDIS planning decisions by providing evidence of the participant’s current needs, capacity, and goals. This ensures that participants receive the right types and levels of support—such as assistive technology, personal care assistance, or therapy services—tailored to their specific circumstances. Ultimately, the assessment empowers participants by highlighting their strengths and identifying areas where targeted support can enhance their quality of life and independence.
NDIS End of Plan or Change of Circumstance Report
Our therapists have extensive experience in NDIS report writing, ensuring all reports meet the specific requirements set by the National Disability Insurance Scheme (NDIS). We are well-versed in the necessary documentation, including detailed assessments, progress reports, and the NDIS End of Plan Reports. These reports detail each participant's progress and future goals. Additionally, we are skilled in preparing Change of Circumstance Reports, which are crucial when a participant's needs or circumstances change. Our team undergoes regular training on how to advocate effectively for a participant's physiotherapy needs, ensuring that all documentation clearly demonstrates how therapy supports them to achieve their goals and enhance their quality of life.
SDA Report Writing
In addition to general NDIS reporting, we also have significant experience in writing comprehensive SDA (Specialist Disability Accommodation) reports. We recognize the importance of these reports in securing the right housing solutions for participants, and we focus on providing thorough documentation that supports the need for appropriate accommodations based on the individual's functional abilities, safety requirements, and long-term goals. Our experience in this area ensures participants can access the optimal living environments tailored to their specific needs.
SIL and SDA Manual Handing and Transfer Plans
Our therapists are highly skilled in creating manual handling and transfer plans for individuals living in SIL (Supported Independent Living) homes. We understand the unique challenges that participants face in these settings, and we provide detailed, practical plans that prioritise safety, independence, and dignity. By working closely with the participant and their support network, we ensure the manual handling strategies are customized to each person’s physical abilities, reducing the risk of injury and enhancing their quality of life.
